First Mate - Given by the Captain of the crew. Not me or anyone else has a word in this decision.
Captaincy - This promotion is only from First Mate, meaning that you must be First Mate before being promoted to Captain. Also this promotion is refered only by your current Captain. I believe Captaincy should be earned.
Ranks - This one is a bit tricky, but keep in mind that hard work will earn you the ranks.
